About Alive And Well
Wound Care

Advanced wound care for seniors and high-risk patients,
delivered at home and in the clinic.

A Practice Devoted To Patients Who Need More Support

Alive And Well Wound Care was created for patients who face wounds that do not heal easily and who need a level of attention that goes beyond routine care. We combine home-based visits with in-clinic appointments to make advanced treatment accessible for seniors and patients with chronic conditions. Every plan is guided by clinicians with focused wound expertise and supported by a team trained to care for medically fragile individuals.

Advantages That Set
Our Care Apart

We deliver hospital-level wound care in the home, including bedside procedures and advanced therapies.
Every plan is guided by providers with dedicated wound expertise, not general home health.
Our model is designed for diabetes, vascular disease, immobility, and recurrent wounds.
Patients see the same specialized team throughout their healing journey.
Referring providers receive updates that help them stay involved in their patients’ care.
We accept Medicare and major insurers to support ongoing treatment needs.

A Mission Rooted In Meaningful Patient Care

Our mission is to offer advanced wound care that feels accessible, supportive, and clinically grounded. We focus on helping seniors and patients with chronic disease avoid complications, reduce hospital visits, and move toward safer healing with consistent guidance from a dedicated clinical team.
